LPMHC - HUD VASH Case Manager

Duties:
Provides outreach and short-term case management services to assigned Veterans and significant others, consistent with the goals and methods of the Healthcare for Homeless Veterans (HCHV) program and with all relevant Medical Center policies and procedures. Case management services are characterized by assertive outreach, individualized treatment planning, a multidimensional orientation and the intensive delivery of services. Conducts outreach to Veterans in shelter as well as to those who may be unsheltered or living in temporary/unfit structures. Assists with hospital discharge planning for Veterans who are homeless or at risk of homelessness. Reviews agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for Veteran placements. Performs site visits of Veteran apartments to ensure that Veterans reside in safe environments, in compliance with local housing codes. Verifies the Veteran's status, eligibility for VA medical care, and eligibility for program participation. Ensures that homeless Veterans are considered as primary participants for program acceptance. Collects and submits HCHV Program participant data, as outlined by the Homeless Operations Management and Evaluation System (HOMES). Coordinates care by serving as an advocate on behalf of Veterans to ensure comprehensive service delivery. Links and refers Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Complies with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the Case Manager. Participates in the HCHV conference calls and attends training offered by the HCHV Team. Follows all local HCHV policies and procedures. This includes following policies and documentation requirements as required for CARF and Joint Commission accreditation. Participates in program planning and development, advises the supervisor of trends with caseloads, identifies needs for program procedure or policy changes and makes recommendations. Takes responsibility for improving professional skills, participating in staff development projects, attending seminars and participating in training programs of other disciplines as requested. Documents social work interventions and activities in Veteran's clinical record utilizing CPRS and ensures appropriate hand off when transferring care. Maintains familiarity and responsibility for all performance measures related to the HCHV program. Engages in outreach activities and provides supportive case management for HCHV Veterans, as needed. Provides case management services to Veterans and significant others in the Grant Per Diem and Contract Residential programs, consistent with the goals and methods of the Healthcare for Homeless Veterans (HCHV) program and with all relevant Medical Center policies and procedures, as needed. United States Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Hold a master's degree in mental health counseling, or a related field, from a program accredited by the Council on Accreditation of Counseling and Related Educational Programs (CACREP). Examples of related mental health counseling fields include, but are not limited to, Addiction Counseling; Community Counseling; Gerontology Counseling; Marital, Couple, and Family Counseling; and Marriage and Family Therapy. A doctoral degree in mental health counseling may not be substituted for the master's degree in mental health counseling. Licensure. Persons hired or reassigned to LPMHC positions in the GS-101 series in VHA must hold a full, current, and unrestricted license to independently practice mental health counseling, which includes diagnosis and treatment. (1) Exception. Non-licensed LPMHCs who otherwise meet the eligibility requirements may be given a temporary appointment as a graduate LPMHC under the authority of 38 U.S.C. 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a LPMHC that provides care under the supervision of a licensed LPMHC at or above the full performance level. This exception only applies at the entry level (GS-9). For grade levels at or above the full performance level, the candidate must be licensed. Please note:
The New York license for LPMHC does not allow the LPMHC to diagnose. Diagnosis is a primary role of the occupation and therefore VHA cannot employ an LPMHC whose only licensure is from New York. Grade Determinations:
GS-11 Licensed Professional Mental Health Counselor (Full Performance Level) (a) Experience, Education, and Licensure. In addition to the basic requirements, candidates must have at least 1 year of post-master's degree mental health counseling experience; OR In addition to the basic requirements, a doctoral degree in mental health counseling, or a related field may be substituted for the required 1 year of professional mental health counseling experience. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
